correction and creation of exams

Hello!
To make it short: On the one hand I'm interested in the creation process of the exams - which you might probably not be able to answer - and on the other hand on what you're looking out for when correcting.
To explain the first further: When creating the IELTS exam what do you look out for that you're able to tell that one who passes a specific band has the skill of the achieved score? Are there specific "grammatical" topics like 3 word-phrasal verbs that one has to bring in in, say, band 7 upwards?
As in regards to the latter: Do you have a special focus on vocabulary - especially in the speaking section - or do you look at it as a whole?
