Common application format for all applications

Currently I have to prepare for IELTS with minimum allocated time per day as I have to do this outside of my work. This is the reason I have decided to use a general format for any application except for job application.
Could anybody please evaluate, if this format is old type or still can be used in IELTS exam?

Also I am not sure if this word "pray" is still used in applications?
